Unnecessary loss

It absolutely amazes me that the officers of the R.M.S. Titanic had the passengers put on life jackets, and directed them to get into the lifeboats. Many of the passengers, while wearing life jackets, chose to stay on the ship. The result was that while the lifeboats could have saved over half of the crew and passengers, around 700 out of 2,200 people were saved.
